My GP has sent me a link to a questionnaire
How to access a Florey Survey for patients
If you have received a text from your GP asking you to complete some questions about your health via a link, you should be able to click on your unique link in that text and complete the survey using your phone if your phone is connected to the internet.
Not able to complete the Florey Survey on your phone?
If you can't use your phone for any reason, you can also still complete the survey on a computer. To do this, type your unique link from the text into the URL search bar of your internet browser.
It should look something like:
florey.accurx.com/eXaMpLe
Some internet browsers may look slightly different to the two shown above but you are usually looking to type your unique link in the search bar along the top of the screen.
Who are accuRx and where has this link come from?
They are a software company providing GP practices with tools to communicate with their patients and other healthcare providers. They are currently used in 50% of GP practices in the country.
If you've received a text with a link in it, this was sent to you individually by a staff member at a GP practice (the bottom of the text should tell you which practice). This would have been sent in order to gather some information from you, and might save an appointment, phone call or letter for both you and the practice.
